returning PatientBean.java to original form with no code smells and pushing code refactoring to a seperate branch

Three code smells identified and fixed. 1- multiple occurrences of the same string literal "MM/dd/yyyy". 2- ageInMs calculation longer than 100 lines. 3- age conversion uses multiple magic numbers.

updated deadPatient SQL files and updated ViewPatientOfficeVisitHistoryActionTest and MedicalReportFilterTest to reflect database changes

Added Selenium tests for different combinations of selections on screen, as well as error scenarios.

Added new FormValidationException to throw "no results returned" when database query results are empty.

Updated testFilterByGender unit test to reflect the addition of three dead female patients. The test was failing as female patients went from 24 to 27.

Updated patients 1-8 to add death date and cause of death data. Also updated TestDataGenerator class to omit calling deadPatient methods as they were causing log-in issues.

Updated causeOfDeathTrendsReport jsp to pass field values (patient, gender, start date, and end date) to DAO class methods. Also modified trends report table by iterating through bean object to retrieve return values from DAO.

Added new CauseOfDeathTrendsReportBean class to persist report results data in bean

Updated cause-of-death trends report to return report results (dates are hardcoded since there is issue with converting string to date)
