Format: delete commented code, change tabs to blank space 

Fixed error so that patients cannot create appointments in the past; fixed style changes; started EditApptPatientTest.java

cancelApptPatient now does not let user submit request to delete appointment more than once; general style fixes

Switched structure of View/Edit Appts to mimic HCP implementation; created editAppPatient.jsp and viewMyApptsPatient.jsp

Updated causeOfDeathTrendsReport jsp to pass field values (patient, gender, start date, and end date) to DAO class methods. Also modified trends report table by iterating through bean object to retrieve return values from DAO.

Updated cause-of-death trends report to return report results (dates are hardcoded since there is issue with converting string to date)
