[UC 92] Activate pre-registered patient

As an HCP, I want to be able to view a list of pre-registered patients and activate the pre-registered patient as a regular iTrust patient so I can edit the detailed information about the patients that we serve.

Tasks:

• The LHCP can view a list of pre-registered patients
• The LHCP can view the detailed information about a pre-registered patient. (Name, Address, Email, Phone, Insurance Information, Height, Weight, Smoker)
• The LHCP can choose the activate the pre-registered patient and the pre-registered patient's account is converted into a regular patient
• The LHCP can click a link to continue to edit the detailed information about the patient
• The LHCP can choose to deactivate the pre-registered patient and the pre-registered patient is then hidden from the list
• All of the above events are properly logged
